Overview
Medical Social Worker in Lawrenceville, Georgia, starting May 11, 2026 for an eight-week travel assignment in an acute care setting. Guaranteed 40 hours per week with a weekly pay range of $1,773 to $1,852. Shift is Monday through Friday, 8:00 a.m. to 4:30 p.m. This is a temporary travel position; local candidates within 50 miles are not considered.

Responsibilities
- Conduct psychosocial assessments for patients and families in an acute care environment.
- Develop and guide discharge planning and care coordination with the multidisciplinary team.
- Provide counseling and resource linkage to address psychosocial needs, including family meetings and crisis support.
- Coordinate referrals to community resources and post-discharge services; document plans and outcomes.
- Maintain accurate, timely documentation in the medical record and ensure regulatory compliance.
- Collaborate with physicians, nurses, case managers, and social work colleagues to support patient disposition and transitions.

Requirements
- Active Georgia LCSW license (GA LCSW) required.
- Minimum 2 years of medical social work experience, preferably in an acute hospital setting.
- Demonstrated experience with discharge planning, patient advocacy, and community resource referrals.
- Ability to work in an acute care setting with daily rounds and multidisciplinary teams.
- This is a travel assignment; no local candidates within 50 miles of Lawrenceville, GA are eligible.
