Rates for submission(strictly follow)

Malpractice Must be included in rates.

Orientation $125-137.50 - half the hourly/callback rate, kicks in after 8 hours. Per hour of orientation or training beyond 8 hours

Hourly $250-275 - Per hour, not OT

Holiday $375-412.50 - Must be 1.5x the hourly rate. Per holiday hour, no OT

Nightly Call $600-700 - Per night call to hold the pager, to include 2.5 hours gratis

Holiday Nightly Call - $900-1050 - "Per night call to hold the pager, to include 2.5 hours gratis

On Call (Daily) $1500-1700 - Per 24 hours to include 4 hours gratis"

24 Hour Holiday Call Rate - $2250-2550 - Per 24 hours to include 4 hours gratis"

Call Back $250-275 Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.

Holiday Call Back $375-412.50 Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.

Schedule/Availability Requirements:

Start: As soon as credentialed

Duration: 6-9 months

Assignment: M-F and Call (1-2 weekdays and weekends)

• Night call starts at 5pm and goes to 7am.

Facility Location: St. Michael Medical Center

Setting: Office/Clinic 5% and Hospital 95%

FTE: 1.0

Duties: Adult 100%

Required Procedures:

• General CV/thoracic surgery open
• Coronary artery bypass surgery on pump
• Valvular repair
• Valvular replacement

Preferred Procedures:

• Aneurysm repair cardiac
• Aneurysm repair aorto-thoracic
• Lung resection (including VATS)
• Chest wall or pleural procedures
• Left ventricular assist devices
• Ventilation management
• Evaluation & management of acute volume/BP issues
• Insertion of central line

NOT Required Procedures:

• • General CV/thoracic surgery endoscopic
  • Coronary artery bypass surgery off pump
  • Congenital defects (ASD, VSD, PFO, etc.)
  • Esophagus and diaphragm surgeries
  • Permanent pacemaker placement
  • AICD (Automatic Internal Cardiac Defibrillator)
  • Bronchoscopy
  • Arterial line
  • PA catheter
  • IABP

• What is the required background/skills? Cardiothoracic Surgery experience - both in the OR and also management of the patients in the acute care and ICU setting

• Admissions - 0-3

• Surgeries 1-2

Support Staff:

• Office ancillary staff: RN Navigators, Mas
• Physicians in practice 2
• Midlevels - 4

Reason for Coverage: Current APP has retired

Travel Preferences:

Preferred Lodging: Oxford Inn & Suites

Air Travel: Local, no flights HIGHLY PREFERRED

Credentialing Information:

Timeframe: 3+ months.
