LPN / RN – Home Care Nurse

Columbia, SC | Statewide Travel Optional

Avodah Home Care , a trusted and rapidly growing home care agency headquartered in Columbia, SC, is seeking a compassionate and dependable LPN or RN to provide in-home nursing services to clients across South Carolina. This is an impactful role for a nurse who is passionate about community-based care, patient dignity, and professional flexibility.

We serve a variety of clients, including aging adults, post-surgical patients, individuals with disabilities, and veterans. Our nursing team plays a vital role in assessing, planning, delivering, and evaluating care that helps clients remain safely and comfortably in their homes.

Key Responsibilities

• Develop and implement individualized care plans under the direction of the physician (RN) or in collaboration with supervising RN (LPN)
• Conduct in-home clinical assessments and regular client reassessments
• Administer medications, wound care, and other skilled services as needed
• Monitor vital signs and observe/report changes in patient conditions
• Maintain accurate and timely clinical documentation and progress notes
• Communicate effectively with physicians, caregivers, and family members
• Supervise home health aides and provide written care instructions
• Educate clients and families on medical conditions, medications, and home safety
• Participate in team meetings, in-service training, and compliance activities
• Respond promptly to urgent care needs and participate in weekend/holiday call rotation if available
• Adhere to infection control, safety protocols, and agency policies
• Maintain client confidentiality and uphold professional conduct at all times

Qualifications

• Active LPN or RN license in the state of South Carolina (required)
• Graduate of an accredited school of nursing
• CPR certification (required)
• 1–3 years of clinical nursing experience (home care preferred)
• Knowledge of home care documentation, compliance, and client safety standards
• Ability to work independently and manage a flexible caseload
• Proficient with EMR systems and comfortable with technology
• Strong communication, organizational, and critical thinking skills
• Valid driver's license, reliable transportation, and current auto insurance
• Must pass a criminal background check and health screening
• In-home client assessment experience strongly preferred

Travel Expectations

While not required, nurses may occasionally be asked to travel up to 100–200 miles across South Carolina to support clients in underserved or high-need areas.

• Travel is optional and based on availability and agency needs
• A minimum gas allowance is provided for mileage outside of the core service area

Pay: $40.00 - $50.00 per hour
